Your Constitution score is 19 while you wear this amulet. It has no effect on you if your Constitution is already 19 or higher

How the Amulet of Health plays

While you wear it, your Constitution score is 19, a +4 modifier. If your Constitution is already 19 or higher, it does nothing. It’s rare and requires attunement.

The payoff is hit points. When your Constitution modifier changes, your hit point maximum changes as though you’d had the new modifier since 1st level. A 10th-level wizard with Constitution 12 goes from +1 to +4 per level: 30 more hit points the moment the amulet goes on. It also improves Constitution saving throws, including the ones that keep concentration going.

Who should wear it

Common questions

How many hit points does an Amulet of Health add?

Your new Constitution modifier (+4) minus your old one, times your character level. From Constitution 10 at 8th level, that’s 32 extra hit points. They go away when you take it off.

Does it stack with raising my Constitution?

No. It sets your score to 19. If an Ability Score Improvement later takes your own Constitution to 19 or 20, the amulet stops doing anything.

Can it raise Constitution above 20?

No. It sets the score to exactly 19. Only the Manual of Bodily Health and a few other rare effects push Constitution past 20.

Does it help concentration?

Yes. Concentration checks are Constitution saving throws, so the +4 modifier applies to every one.

Is the Amulet of Health worth an attunement slot?

For a character with Constitution 14 or less, usually yes: it’s a large, permanent pile of hit points and better concentration. For one with 16, it’s +1 to the modifier, and other rare items may do more.

How much is an Amulet of Health worth?

It’s rare. The Dungeon Master’s Guide puts rare items at 501 to 5,000 gp, and the builder’s Standard Conversion prices them at 5,000 gp.

Gauntlets of Ogre PowerUncommon: sets Strength to 19.
Manual of Bodily HealthVery rare: +2 Constitution and +2 to its maximum, permanently, with no attunement.
Ioun Stone of FortitudeVery rare: +2 Constitution, but only up to 20. Better if your Constitution is already 18.
